Description

A 360-degree feedback survey is a tool used to collect information from various sources about an employee’s performance or behavior. These sources include, but are not limited to:

  • Colleagues
  • Supervisors
  • Clients
  • Other stakeholders

This feedback collected can be used to evaluate job performance and identify potential areas for growth and development.

How to Use It

This template can be customized to assess key competencies that fit the needs and objectives of your organization. 

The competencies listed and questions provided are not exhaustive—you should add/remove competencies and relevant questions as needed. Questions ought to be presented in an open-ended way so respondents can offer additional details and insight. 

Once your survey is ready to be used, distribute the surveys to respondents, including instructions on how to fill out and submit the survey.

Ensure respondents understand how your chosen rating scale works (i.e., 1-5, 1-10, etc.) and remind them that they have the opportunity to include additional comments at the end of each section.

When survey results have been submitted, organize and compile the feedback and share results with the employee being evaluated. The employee can use the insights to identify areas for growth, improvement, and specific actions they can take to improve in the relevant areas.
